You must initiate the process by applying to student teach.  For information on the student teaching application, including eligibility requirements and deadlines, click on the application link to the left.

If we have contacted you to inform you that you will need to complete a Limited or Full Criminal History Check for your student teaching placement, you may download the form(PDF); you may also complete the form online, but you cannot submit the form online. If you complete the form online, you must print the form and complete the process either by mail or in person. Be sure to read the form for complete instructions.

If you are absent for any reason during your student teaching experience, please complete the Request for Absence from Student Teaching form (PDF).
