Complete six
hours of academic credit at Purdue University in the teaching major, minor,
or endorsement area(s) or in professional education within five years of the
application date. A grade point average of 2.5/4.0 is required for renewal. |

2.
|
For Rules 2002
license renewal, the Professional Growth Plan (PGP) must be completed.
Submit the appropriate documents no less than 120 days prior to the
expiration of your current license to the Division of Professional
Standards. Individuals with a Bulletin 400 or Rules 46-47 license may use
the PGP to renew those licenses; however, all subsequent renewals must be

Hearing Clinician license, contact a licensing advisor for specific
requirements. |

Request and
receive an evaluation for the addition. If an evaluation has not been
completed, the evaluation will be done at the time of application, and the
applicant will be charged a minimum of $35 for the evaluation. A cashier’s
check or money order payable to Purdue University is required for the
evaluation. |

Complete the
required course work at Purdue University, maintaining a 2.5/4.0 index. |

The application
fee for an addition and renewal at the same time is $35. |
- CONVERSION TO A
PROFESSIONAL LICENSE

LIMITED CRIMINAL HISTORY

Complete the
personal information at the top of the Request for Adult Criminal History
Information Form #8053. The applicant is the subject of request and the
requesting individual. Therefore, the report needs to be returned to you
from the Indiana State Police. Do not request that the form
be mailed to the Office of Professional Preparation and Licensure. The Box
#2 under the $7.00 section should be marked. Sign and date at the bottom
of the second page. |

Submit
completed Form #8053 and a cashier’s check or money order for $7.00 payable
to the State of Indiana to the Indiana State Police, Central
Repository, Indiana Government Center North, 100 North Senate Avenue, Room
N302, Indianapolis, IN 46204-2259 (317-233-5424). |

Results will
be returned to you in three to four weeks. The limited criminal history
report is valid for a period of one year from the date of issuance
regardless of the number of license actions requested within that time
frame. |

A limited
criminal history report can also be downloaded from the Internet at
http://www.in.gov/isp. This service costs $16.32. Under Reason for
Request, check #2, “Applicant has applied for a license and criminal
history data is required by law to be provided in connection with the
license.” The set out authority number to add for Reason #2 is:
IC-20-6.1-3-7.1. Be sure to print the report. |

If you are
currently working in a school setting, your school corporation may be able
to download a limited criminal history report for its employees from the
website listed in #4 above. A printout on the on-line limited criminal
history report is acceptable for submission with your license materials. |

not send a photocopy. If you have already submitted a limited
criminal history report for another license action within a year,
please make a note of this on the materials you submit to this
office so your application will not be returned to you as
incomplete. |

Include an official transcript if any course work for a renewal,
addition, or conversion has been completed at another university
(unless already submitted for an evaluation). |

Include a valid license (A copy may be submitted if the license has
expired. If you have completed the Beginning Internship Program,
however, the license with original signatures on the back of the
license must be submitted). |

6.
|
For a
conversion, submit a letter verifying five years of teaching
experience on school corporation letterhead signed by the personnel
office of the school corporation. Dates of employment, position
held, grades/subjects taught or administered, whether the experience
was full or part time, and the accreditation status of the school at
the time of employment should be included. |
