  • To cancel a paper-based test and request a refund, you must submit a refund request form to ETS.  This form must be received no later than the late registration deadline for that test date. To cancel a computer-based test, you must call the appropriate test center no later than three days prior to your appointment or your test fee will be forfeited.  Registration fees are not refundable.
  • You must have acceptable and valid ID with signature and photograph to be admitted to a test center.  ID requirements are strictly enforced.

    Score reports
  • Scores must be released to Purdue University West Lafayette at the time of registration; this is the Designated Score Recipient(s) section.  Enter Purdue WLAF online or write in Code#R1631 on the registration form.  (The Attending Institution also is Purdue WLAF or Code#A1631.)
  • If you do not release your scores to Purdue University, the Office of Professional Preparation and Licensure will not be able to process your application for the Teacher Education Program or licensing. Scores will need to be reordered through the Educational Testing Service. The cost is $35.
